ELIGIBILITY QUESTIONS

A) Do you suffer from any physical or mental disorder, are you a consumer or drug addict, or do you currently have any of the following diseases (contagious diseases are specified in accordance with section 361 (b) of the Public Health Service Act):

      - Diphtheria
      - Tuberculosis, infectious
      - Plague
      - Smallpox
      - Yellow fever
      - Viral hemorrahagic fevers, including Ebola, Lassa, Marburg, Crimean-Congo
      - Severe acute respiratory illnesses capable of transmission to other persons and likely to cause mortality.

B) Have you ever been arrested or convicted of a crime that will cause serious damage to property, to another person or to some government authority?


C) Have you ever violated any law related to the possession, use or distribution of illegal drugs?


D) Are you looking to participate or have you ever participated in terrorist activities, espionage, sabotage or genocide?


E) Have you ever committed fraud or lied to obtain, or help others obtain a visa or entry to the United States?


F) Are you seeking employment in the United States or were you previously employed in the United States without prior permission from the United States government?


G) Have you ever been denied a visa to the United States that you have applied for with your current or previous passport, or have you ever been denied admission to the United States, or have you withdrawn your application for admission to the United States into a port of entry? entry?
